Two simple ways to use:

1. Simply "copy and paste" the order form to a Word document and then when you have completed it, email it to us at  debbie@vincentandvincent.net, or

2. Print the order form and when complete fax it  to 615-451-9019!

We will complete it with your total including shipping, and processing, if applicable, and fax it or email it back for confirmation!  You can include any of the publishers we represent and we will separate the orders if necessary.
